Transition from School to Community
About this Service
If you’re in your final year of school and have been assessed as High or Very High needs under the Ongoing Resourcing Scheme (ORS) we may be able to help as a Transition from School provider. We can help build a personalised plan and support you in all aspects of life, including daily living, social life, work and skills, cultural and community engagement.
Transition service can assist with:
- Considering all post-school employment, education and community participation options
- Developing an individual plan for realising goals for life after school
- Opportunities to participate in activities, services and facilities of interest
- Developing skills for everyday life e.g. work experience, volunteering, culture awareness
- Identifying appropriate employment opportunities
- Gaining and maintaining paid employment or self-employment
- Maintaining and extend friendships and social and support networks
Working with you, your whānau and your school, we will look at what you want to achieve within your life after school, and what steps or supports may be required to enable you to achieve this.
We will create a person directed pathway plan which focuses on your wants, needs, aims and aspirations. All the things that matter to you.
We work across Hawkes Bay; therefore, we can meet you somewhere that you are comfortable with, and as often as you need us to.
We can even go with you as you trial your next steps, providing you with that little extra help you may need, be it directly or just a bit of moral support.
