Transition from School to Community
About this Service
If you’re in your final year of school, and have high needs for support next year, we may be able to help as a Transition from School provider. We can help build a personalised plan and support you in all aspects of life, including daily living, social life, work and skills, cultural and community engagement.
This can include:
- Developing an individual plan for realising goals for life after school
- Opportunities to participate in activities, services and facilities of interest
- Developing skills for everyday life e.g. work experience, volunteering, culture awareness
- Identifying appropriate employment opportunities
- Gaining and maintaining paid employment or self-employment
- Maintaining and extend friendships and social and support networks

Getting Involved
You can make a difference in someone’s life by getting involved with the Disability Resource Centre. You can do this with a donation or by simply volunteering your time.
